What we collect

If you send us a question through the form, we receive what you enter: your name, your email address or phone number, and your question. Please don’t send health information through the form. For appointments, prescriptions, test results or anything urgent, call us on 09 298 0778.

Where it goes

The form sends your question to the practice by email. The website itself doesn’t store it. The site is hosted by Cloudflare, which processes the request on our behalf, and Cloudflare Turnstile checks that the form is being sent by a person, not a bot.

How we use it

We use your details only to answer your question. We don’t add you to a mailing list, and we don’t sell or share your details.

Cookies and tracking

This website doesn’t use cookies to track you, and it has no advertising or analytics trackers.

Your health information

As a health provider, we must follow the Health Information Privacy Code 2020. It sets rules for how we collect, use, store and share health information about you.

• We collect health information to care for you, and add to your record each time we see you.
• We share relevant information with other health professionals who are directly involved in your care.
• Your enrolment details go to our PHO, the National Hauora Coalition, and to the government, so your visits can be subsidised.
• If you’re in a health programme, such as screening, immunisation or diabetes care, relevant information may go to the organisation that runs it.
• Otherwise, details about your health and the care you receive stay confidential within the practice unless you agree to share them.

Your rights

You can ask to see the health information we hold about you, and ask us to correct it. Ask at reception, or contact our privacy officer.
